NOMINATION FORMS REQUIREMENTS • Employees are eligible to be nominated for the individual awards only after completion of minimum 12 months of service at the Al Habtoor Group in the last calendar / academic year. • All individual nominations should be cleared by the unit’s HR for having no-warnings in the past calendar / academic year. • All nomination forms submitted must be scored. Forms that are not scored will be automatically disqualified. • Individual nominations should be accompanied by the Employee Performance Appraisal forms (not older than 3 months). The Employee Appraisal constitutes 70 points of the score of the Individual Excellence Award. • A clear description should be provided for every criteria in the form. The description should be between 100 to 200 words. • Description should be clearly typed on the computer, not handwritten. • All forms need to be initialled by the local AWC members and local auditor prior to presenting them to the head of the business unit. • No nominations will be accepted after the submission deadline unless otherwise approved by the Awards Board and the Audit Board. • A winner cannot be nominated for the same award two years in a row. • Nomination forms for employees who resigned should be disqualified, removed. • Business unit must present nominations at least twice the number of awards allocated for that unit under the Individual Excellence Award. 62
